You can unlock three gamer icons that can be used in your Xbox Live profile...

1) Complete the game on Insane difficulty, to get Insane difficulty you first need to complete the game on casual or hardcore.

2) In ranked Xbox live matches get 10,000 kills.

3) Finish a ranked versus match
